As the owner of an original scala-VoIP you are entitled to a $15.00 Cash Rebate if you decide
to purchase a scala-rider headset. Please note: You must be in the possession of both products
in order to receive your Cash Rebate. This right is transferable but the party you wish to transfer
it to must be in the possession of the sales slip for the scala-VoIP and of the form below.
How to claim your Cash Rebate: Once you have purchased a scala-rider headset, you must
either fax or send by registered mail the sales slip of both products, i.e. the sales slips for the
scala-VoIP and the scala-rider. Along with the sales slips you must send us (by fax or mail) the
filled form below.
