1 Notifioadon to the Local Telephone Company
On the bottom of this equipment isa label indicating,among other information,the FCCRegistrationnumberand
RingerEquivalenceNumber (REN)for the equipment.You must, upon request, providethis informationto your
telephone company.
The REN is usefulin determining the numberof devicesyou may connectto your telephone line end still haveall
of these devices dng when your telephone number is celled, in most (but not all) areas,the sum of the RENsof all
devicesconnected to one line shouldnot exceed 5. To becertainof the numberof devicesyoumay connect to your
line as determinedbythe REN, you shouldcontactyour localtobphone company.
Notes
• This equipment may not be usedon coin serviceprovided by the telephone company.
• Party linesare subject to state tariffs, and therefore, you may not be able to useyour own telephone equipment
if youare on s party line. Checkwith your localtelephone dompany.
• Noticemust be given to the telephone company upon permanent disconnection of your telephone from your
line.
2 Rights of the Telephone Company
Should your equipmentcausetrouble on your line which may harm the telephone network,the telephone
company shall, where practicable, notifyyou that temporary discontinuance of servicemay be required. Where
prior noticeis not practicableand the circumstances warrant suchaction,the telephone company may
tomporadly discontinueservice immediately.In case of suchtemporary discontinuance, t he telephone company
must: (1}promptly noffly you of suchtemporary discontinuance; ( 2) afford you the opportunityto correctthe
situation;and (3} inform you of your right to bring a complaintto the Commissionpursuantto proceduresset
forth in Subpert E of Pa_ 68, FCCRulesand Regulations.
The telephone company may make changesin itscommunications fscilities,equipment, operationsof
procedureswhere such actionis required in the operationof itsbusinessand not inconsistentwith FCCRulesand
Regulations.if these changesare expectedto affectthe use or performance of your telephone equipment,the
telephonecompany must giveyou adequatenotice, i n writing,to allowyou to maintain uninterrupted service.
